REGISTRATION FEES

Please note that all participants, including presenters, are required to register and pay the registration fee. The registration fee will be for the Campaign'99 kit, lunches, welcome dinner and abstracts.

PAYMENT MODES

Payment can be made through Banker's Cheque, Bank Draft or Telex Tranfer to Malaysian Council for Rehabilitation. Personal cheques will not be accepted (except for local participants).

 
ACKNOWLEDGEMENT OF FEES

Upon receipt of your completed registration form and fees, the Secretariat will send you an acknowledgement letter. Please present the acknowledgement at the Registration desk in order to collect your conference materials.

 
CANCELLATIONS AND REFUNDS

In the event that pre-registered participants are unable to attend, their fees will be refunded, less USD 50 as administration fee, unless written notice of non-attendance is received two months prior to the Conference. The hotel deposit will be withheld as all refunds will be processed after the Conference.

 
ACCOMMODATION AND CONFIRMATION

All reservation requests will be sent to the Conference Secretariat. Special conference rates are available only through the Secretariat. Participants are advised to stay in the Conference venue for the entire Conference. A hotel deposit for one night shall be paid in advance, together with the registration fee. After receipt of payment, you will be sent a confirmation of registration and hotel reservation by the Conference Secretariat. Reservations by telephone will not be accepted, but you may make your reservation by fax.
